MyUPMC Pediatric Proxy

Pediatric Proxy allows parents/legal guardians access to their child’s health record through MyUPMC. Pediatric Proxy is available for parents/legal guardians from the birth of their child until the child’s 18th birthday.

How to obtain Pediatric Proxy for MyUPMC members:

  • Complete the online Pediatric Proxy request form under “Manage Another Family Member's Account” on MyUPMC.
  • Once your request has been processed and approved, you will be granted access to your child’s record through MyUPMC and will receive an email confirmation from Children’s Community Pediatrics.
  • To active your Pediatric Proxy account, please log in to your MyUPMC account and click on your child’s name from the dropdown menu.
  • Read the Proxy disclaimer pop-up and select continue.
  • Once in the child's account, click on “Review Your Messages.” Your proxy access will become active by clicking on any menu option within your child's account.

How to obtain Pediatric Proxy for Adolescents:

To protect the privacy of adolescent patients, Pediatric Proxy access automatically changes the night before a child’s 13th birthday to Limited Teen Access. Full Teen Access can be granted at the adolescent’s request through a conversation with their provider.
