Understanding Pediatric Proxy for adolescents:

To protect the privacy rights of adolescent patients, Pediatric Proxy access for parents/ legal guardians automatically changes the night before a child’s 13th birthday to Limited Proxy Access. This means a proxy holder will now only be able to see the following aspects of their child’s health record.

Limited Access

  • Immunization records
  • Allergies
  • Upcoming appointments
  • Medical advice requests to PCP
  • Growth charts
  • Glucose or blood pressure tracking tools
  • Eyeglass/contact prescription information
  • Contact and administrative information
  • Health insurance information

Your access can be changed back to Full Proxy Access at the adolescent’s request through a conversation with their pediatric provider. With Full Proxy Access you will be able to view the items listed above and below within your child’s health record.

Full Access

Everything viewable in Limited Access plus:

  • Medication list
  • Medication refill requests
  • Current health issues
  • Medical history
  • Test results
  • Recent visit summary
  • Medical advice requests to specialists
  • Appointment requests to specialists
