By functioning as your Primary Care Medical Home, UPMC CCP – Altoona puts your child at the center of health care decision-making. Your chosen pediatrician, along with the rest of our office staff, will work in partnership with your family to coordinate and personalize care. Your provider will also work with you and your child to establish a unique care management plan.
